Eligibility

To be eligible for service coordination services an individual must meet the following criteria as determined by the Department of Mental Health:

1. Have a physical or mental disability that was diagnosed before age 22.

2. The disability is likely to continue indefinitely.

3. The disability results in major limitation in two of the following areas:
